Nurse Practitioner

Secaucus, NJ

Tristate Health Pay, LLC is seeking a Nurse Practitioner to provide comprehensive, evidence-based primary and preventive care to adult patients in a Tristate internal medicine practice in Secaucus, NJ with support in North Bergen, NJ. In this role, you will independently manage care within the scope permitted by applicable law and organizational privileges while collaborating with physicians and the interdisciplinary team to promote strong clinical outcomes, continuity of care, and excellent patient experience.

What We can offer YOU:

• Pay: $144,000 – $173,000/year, offer dependent on experience

Schedule:

• Monday through Friday
• 8:30AM to 5PM EST
• Potential Saturday coverage as necessary
• Full time

The right candidate will have:

• Master's or doctoral degree in nursing from an accredited Nurse Practitioner program.
• Higher education may be substituted for experience.
• Two or more years of advanced practice experience in internal medicine, adult primary care, family medicine, or a comparable outpatient setting.
• Experience managing medically complex adults with multiple chronic conditions
• Experience with value-based care, quality metrics, population health, precepting, or clinical workflow improvement.
• Current, unrestricted Nurse Practitioner/APRN license in each state in which services are provided, or eligibility to obtain before employment.
• Current national board certification appropriate to the adult primary care population, such as Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP).
• Current prescriptive authority and DEA registration, when required for assigned duties, or eligibility to obtain within the timeframe established by Tristate.
• Patient-centered service, empathy, and respect for diverse backgrounds and needs.

Our Nurse Practitioner will:

• Collaborative Physician-APP Environment: Work within a supportive, team-based practice where the NP and physician collaborate on patient care, clinical decision-making, and complex cases.
• Clinical Autonomy & Support: Maintain appropriate autonomy in evaluating and managing patients within the NP’s licensure, training, privileges, and scope of practice, with access to physician consultation when clinically indicated.
• Comprehensive Patient Care: Conduct comprehensive and focused assessments, obtain histories, perform examinations, and manage acute, chronic, and preventive health conditions in adult patients.
• Evidence-Based Treatment: Develop, implement, and evaluate individualized treatment plans using current clinical guidelines, appropriate diagnostic testing, medications, and therapeutic interventions.
• Chronic Disease & Preventive Care: Provide ongoing management of chronic conditions, annual wellness visits, preventive care, episodic illness, and post-hospital/transitional care.
• Accessible Physician Consultation: Have a physician who is available and approachable for consultation, particularly for complex, urgent, or higher-risk clinical situations, without unnecessarily limiting the NP’s day-to-day clinical judgment.
• Efficient Clinical Operations: Maintain a manageable and well-organized patient schedule, clinical inbox, and workflow that supports quality care without sacrificing patient safety, documentation, or patient experience.
• Strong Care Coordination: Collaborate effectively with physicians, specialists, clinical staff, and other healthcare professionals to facilitate referrals, transitions of care, and continuity of treatment.
• Patient-Centered Communication: Prioritize patient and family education, shared decision-making, medication counseling, preventive health, self-management, and positive patient relationships.
• Appropriate Clinical Resources: Have access to necessary laboratory, imaging, point-of-care testing, procedures, EHR functionality, clinical staff, and other resources needed to provide effective patient care.
• Professional Growth & Practice Improvement: Participate in quality improvement, population health, patient safety, value-based care, and professional development opportunities while maintaining current clinical competencies.
• Compliance & Professional Standards: Operate within applicable clinical, privacy, infection-prevention, prescribing, billing, documentation, regulatory, and organizational requirements while maintaining accurate and timely EHR documentation.
